We have removed the forms from our website as you can now send us messages from your customer service center.
If you need to send us a message for anything including Suspensions, Vacations or Withdrawals you may now log into the customer service center and at the top of your account page you will see a drop down box with the label

 

"I want to:"

please use the drop down box and choose any of the items to send us a message; if you do not see what you want then please use the general inquiry and we will process your request. When requesting a vacation, suspension or withdrawal we will send you a verification email.

Make-up Lesson Request with Doctors Note

We have made fundamental changes in our swim lessons and can no longer accommodate make up requests. Our new student teacher ratios create both a higher quality lesson and a closer relationship between our swimmers and their teachers.  When we made the change to a lower ratio we lost the makeup space.  We are sorry this lower ratio lesson system leaves no room for booking in makeup swimmers.

We will fill make up requests for two or more absences with a doctor’s note only space permitting.  

Please fax your doctor’s note for two or more absences to 954.603.1500 When going on vacation please fax a vacation form to www.us-swim.com.
